Complaints Management & File Movement Digitisation

1 · Purpose of this pack
The Office receives complaints concerning cases whose files are held in paper form across three institutions. Neither the Office nor the complainant is presently able to establish where a file is, how long it has been there, or who last held it. This pack sets out a proposal to address that condition in stages, beginning with the movement of files and the recording of complaints, and deferring wider digitisation until those two foundations are demonstrably working.
The condition addressed Twelve paper records across three institutions, reconciled by hand. A file in transit is invisible to both the sending and the receiving office until it is physically entered in a register at the destination.
The proposal A barcode on each file jacket scanned at every handover, a complaint facility at each stage of the chain, and role-based access for named officers. The paper record remains the record of truth throughout.
The evidence offered Three real-form cases — Jinja Road, Arua and Masaka — traced from station diary entry to judgement, with every scan, every derived delay and every alert the system would have raised.
2 · Scope discipline and measurable targets
The programme is deliberately narrow. Each wave is permitted to begin only when the preceding wave has met its gate. No wave proceeds on the basis of a calendar date alone.

Primary target 20% Reduction in slow response to complaints Measured as the proportion of complaints exceeding the determination standard of 45 days, compared against a baseline established during the first 30 days of the pilot. A baseline must be recorded before the target can be claimed.
Directional objective Zero Untracked file movements Every physical handover produces a scan at despatch and a scan at receipt. Movements without a matching pair are reported weekly as a coverage figure, and no analysis is published while coverage remains below 80 per cent.
Reporting objective Weekly Position known without enquiry The Office is able to state the location and age of any pilot file without telephoning another office, and the Directorate receives a standing weekly report rather than commissioning one.

A caution for the presenter The data in the prototype is illustrative. The three cases are constructed to be realistic in form and are not drawn from actual files, and the staff names are taken from published sources for the purpose of the demonstration only. Both points should be stated at the outset, so that no figure presented is mistaken for a measurement of current performance.
